1. What is your permanent residential address and contact details?
→現在の住所と連絡先を入力。
2. How long have you lived at this address?
→上記の住所に住んでいる期間を入力。
3. Are your preferred details different from the contact details already provided above?
→上記と異なる連絡先を希望するか。Yes/Noを選択。
Travel History
1. Have you been issued any visa for the UK, UK Overseas Territories or Commonwealth Country in the last 10 years?
→過去10年間にイギリス等でビザを発行されたことがあるか。Yes/Noを選択。
2. Have you ever traveled to the UK in the last 10 years?
→過去10年間でイギリスに行ったことはあるか。Yes/Noを選択。Yesの場合は直近のイギリス訪問についての情報、Date of Arrival（訪問日）、Departure date（帰国日）、Purpose of the trip（目的）を記入。私は旅行が目的だったので、 Travelと記入しました。5個まで記入できますので、それ以上の方はプリント後に手書きで記入します。
3. have you made an application to the Home Office to remain in the UK in the last 10 years?
→過去10年間にビザの申請をしたことがあるか。Yes/Noを選択。
4. Have you been refused entry to the UK in the last 10 years?
→過去10年間でイギリスへの入国を拒否されたことがあるか。Yes/Noを選択。
5. Have you been refused a visa for any country including the UK in the last 10 years?
→過去10年間でイギリスを含む国においてビザの発行を拒否されたことがあるか。Yes/Noを選択。
6. Have you been deported, removed or otherwise required to leave any country including the UK in the last 10 years?
→過去10年間でイギリスを含む国で、強制送還、国外退去、出国要請を受けたことがあるか。Yes/Noを選択。
7. Have you ever voluntarily elected to depart the UK?
→自分の意思でイギリスを出国したことがあるか。Yes/Noを選択。
8. Are you, or have you been subject to, an exclusion order from UK?
→イギリスから入国拒否されているか。または、されたことはあるか。Yes/Noを選択。
9. Have you ever traveled outside your country of residence, excluding the UK, in the last 10 years?
→過去10年間に、イギリスをのぞく国へ行ったことがあるか。Yes/Noを選択。Yesの場合は、Date of Travel（訪問日）、Country visited（訪問した国）、Reason of the trip（目的）を記入。私は旅行が目的だったので、 Travelと記入しました。10個まで記入できますので、それ以上の方はプリント後に手書きで記入します。
10. Do you have a UK National Insurance Number?
→イギリスの国民保険番号を持っているか。 Yes/Noを選択。Yesの場合は番号を入力。
11. Do you have any criminal convictions in any country（including spent/unspent convictions and traffic offences）?
→いずれかの国で有罪判決（前科/時候なし前科や交通違反を含む）を受けたことがあるか。Yes/Noを選択。
12. Have you ever been charged in any country with a criminal offence for which you have not yet been tried in the court（including traffic offences）?
→いずれかの国でまだ裁判を受けていない犯罪で起訴されたことはあるか（交通違反も含む）。Yes/Noを選択。
13. Have you ever been involved in, supported or encouraged terrorist activities in any country? Have you ever been a member of, or given support to ,an organisation that has been connected with terrorism?
→いずれかの国においてテロ活動に関与したり、支援、奨励をしたことはあるか。テロ活動をしている組織のメンバーであったりその組織を支援したことはあるか。
14. Have you ever, by any means or medium, expressed views that justify or glorify terrorist violence, or that may encourage others to commit acts of terrorism or other serious criminal acts?
→ 何らかの手段やメディアを通じてテロリストの暴力を正当化したり、他者にテロリスト行為や重大な犯罪行為を奨励するような考えを表明したことがあるか。
15.In times of either peace or war have you ever been involved in, or suspected of invilment in, war crimes, crime against humanity or genocide?
→戦争犯罪や非人道的犯罪、または大量虐殺に加わったり参加を疑われたことはあるか。
Yes/Noを選択。
16. Have you engaged in any other activities that might indicate that you may not be admitted to the UK?
→イギリスにとって相応しくない活動に加担したことはあるか。Yes/Noを選択。
17. If we need to interview you, what language would you like to use in the interview?
→面接が必要になった場合、どの言語で面接を希望するか。希望の言語を記入。私はJapanese, English.
